LIABILITY WAIVER – ALIGN FITNESS STUDIOS

Assumption of Risk

I, the undersigned, acknowledge that participation in Hot Mat Pilates and Reformer Pilates classes at Align Fitness Studios involves physical activity that carries potential risks of injury. These risks include, but are not limited to: muscle strain, sprains, dehydration, dizziness, fainting, falls, or other physical injuries. I understand that Hot Mat Pilates classes are conducted in a heated environment, which may further increase risks associated with elevated heart rate, heat exhaustion, or dehydration.

I voluntarily choose to participate in these activities and assume full responsibility for any and all injuries or damages that I may sustain as a result of my participation, whether arising from the negligence of the instructors, staff, or other participants, or otherwise.

Medical Clearance

I affirm that I am physically capable of participating in these fitness activities and have consulted with a physician if necessary. I agree to inform the instructor of any medical conditions, injuries, or physical limitations prior to class. I understand that it is my responsibility to stop participating if I experience any discomfort, pain, or health concerns during class.

Release of Liability

In consideration of being permitted to participate in classes, programs, or use of facilities at Align Fitness Studios, I hereby release, waive, and discharge Align Fitness Studios, its owners, instructors, employees, and agents from any and all claims, demands, damages, or causes of action arising out of or connected with my participation in any activity, including personal injury, disability, property damage, or death.

Indemnification

I agree to indemnify and hold harmless Align Fitness Studios and its staff from any loss, liability, damage, or cost they may incur due to my participation in classes or use of equipment.

Use of Facilities and Equipment

I understand that proper form, alignment, and control are essential when using Pilates reformers and other studio equipment. I agree to follow all instructions given by instructors and use the equipment responsibly.

Hot Mat Cancellation Policy

I understand that if I do not cancel my hot mat class within a 6 hour window I will be charged a $15 dollar fee and if I no show it will be a $20 fee. I agree to pay this penalty.

Reformer Cancellation Policy

I understand that if I do not cancel my hot mat class within a 6 hour window I will be charged a $20 dollar fee and if I no show it will be a $25 fee. I agree to pay this penalty.

Photography and Media Release

I give permission for Align Fitness Studios to use photographs or videos taken during classes for promotional or marketing purposes.

Acknowledgment

I have read this waiver and release, understand its terms, and sign it voluntarily. I understand that by agreeing, I am giving up substantial legal rights, including the right to sue Align Fitness Studios for negligence.
